Full Job Description

Managing your own clinical caseload, you will see patients at outpatient clinics, in care homes and in patient's own homes, in groups and also offer virtual consultations (telephone and video) where appropriate.
You will be joining our experienced and supportive team of 15 primary care dietitians. You will be able to participate in the peer support sessions and 1:1 supervision to facilitate clinical excellence and personal development. We actively participate in clinical audit, student training and clinical service developments.
Whilst this position is full time, we would consider applicants who wish to job share or work reduced hours in this role.
The successful candidate will have experience of providing dietetic care for patients with a range of clinical conditions and will be able to demonstrate an enthusiasm for working in a diverse Primary Care setting. Communicating evidence-based personalised dietetic advice in an engaging style will be a key component of this position.

Closing date: 20/09/2024, Provision of specialist Dietetic assessments and treatment to patients at health centres and a domiciliary service to patients in their own home and Care Homes within Brent.
· Management of specialist caseload of clients who are malnourished or enterally fed and to maintain associated patient records.
· Making recommendations to GPs on appropriate prescribing for their patients' dietary requirements.
· Liaising with Dietitians and other health care professionals to ensure clients discharged from secondary care are followed up appropriately and receive ongoing input as required following discharge
· Participating in clinical governance including Quality Improvement activities within Nutrition & Dietetics Brent.
