These vacancies offer a position for an experienced registered mental health nurse to work in an exciting, innovative post that supports patients to access mental health support via Primary Care. It is important that candidates have community experience and understand the role.

  • This job is only suitable for an NMC Registered Mental Health Nurse.
  • The core working hours of this role are Mon - Fri, 9am - 5pm excluding bankholidays. Applicants will need to confirm they are able to complete these hours and have the means to be able to travel between the different GP surgeries. The successful applicant will work in the Primary Care setting, offering specialist Mental health assessment to those patients who have moderate to severe mental health conditions.Main duties of the job Please ensure you state your preferred area to work on your application. Central & West Vacancy Central & West Warrington PCN is formed of 7 GP Practices which are unique and individual, sharing the common goal of working together to improve health of and outcomes for our patient population. Our Clinical Director is supported by a Senior Manager and Digital & Transformation Lead to ensure we are highly organised, proactive & innovative. Our MHP's play a key part in our PCN multidisciplinary team working alongside our Pharmacy team, Social Prescribing Link Workers, Health & Wellbeing Coaches and Dietitian to provide a holistic approach to personalised care. We offer staff a programme of 10 Protected Learning Time (PLT) sessions per year. Our dynamic PCN has also been nominated for multiple awards for innovative ways of working. We are keen to expand our Mental Health offer to support the patients of Central & West Warrington PCN. Central East Vacancy Central East PCN is one of five PCNs within Warrington, Cheshire. We provide primary care services to 28,565 patients, across five GP practices. With the creation of the 2020/21 Network Direct Enhanced Services (DES) and the introduction of the NHS Additional Role Reimbursement (ARRS) to support primary care, the PCN has recruited staff in various disciplines

Central & West - covering Eric Moore, Causeway, Dallam, Penketh, Helsby Street, Folly Lane and Westbrook.
