Key Areas of Responsibility

  • To assess and prescribe care packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital/Reablement and Recovery beds.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.

  • An exciting opportunity has arisen for enthusiastic and motivated individuals to join Norfolk Community Health and Care NHS Trust with the Reablement and Recovery Beds and supporting as required on Alder Ward. We are looking for an experienced and reliable Physiotherapist to join us in providing high quality therapy service. The role is to assess patients including those with a complex presentation, using investigative, analytical and clinical reasoning skills and formulate and execute individualised physiotherapy management and treatment plans of patients. You will participate in multidisciplinary team meetings including liaising with GPs, Social Services, other allied health care professionals and the voluntary sector to ensure identified needs are met and care is co-ordinated appropriately. Ideally you will be familiar with these clinical databases; Systmone, TCES, Liquid Logic though training will be provided. As a therapist it is essential that you can prioritise your own workload, possess effective communication skills, work as part of a team, and use your own initiative. If you thrive on variety in the working day, enjoy multi-tasking and also being part of a supportive team then this role is for you. The service is operational Monday to Friday 09.00 - 17.00.
