Please see attached full job description and person specifications Clinical responsibilities Provide a full range of medical services as defined in the core GMS contract and additional enhanced services or PCN services where appropriate In accordance with the practice timetable, as agreed, the post-holder will make him/her self-available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, video consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation Assessing the health care needs of patients with undifferentiated and undiagnosed problems Screening patients for disease risk factors and early signs of illness Develop care plans for health in consultation with patients and
in line with current practice disease management protocols Providing counselling and health education Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate Recording clear and contemporaneous consultation notes to agreed standards using SystmOne Collecting data for audit purposes Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate Undertake other work which is deemed necessary for patients registered at the Surgery, including but not exhaustive, medical reports, letters, forms and other medical administration within core Surgery hours. Behave in a professional and courteous manner to all members of the Practice team, including GP colleagues, Practice team and members of the WSFT Primary Care Team. In general, the post-holder will be
expected to undertake all the normal duties and responsibilities associated with a GP working within primary care. Job plans will be agreed with the post holder and these will be in line with current arrangements. Virtual integration (VI) is an innovative venture supported by the West Suffolk Clinical Commissioning Group and local Alliance which provides support to the GPs and surgery staff from the local acute hospital trust and community services. We believe clinicians working together are in a better position to deliver effective, high quality clinical care. This has allowed staff access to services such as: Occupational health, MSK Physiotherapist, Staff Wellbeing service. There is also currently a free gym membership with Abbeycroft gyms in the West Suffolk area. Glemsford village is within easy reach of Cambridge (30 miles), Ipswich (25 miles), Bury St Edmunds (12 miles) and Sudbury (6 miles) and the surgery benefits from a co-located but independently managed pharmacy,
enabling clinical and patient access to in-house help and advice for medication issues. West Suffolk Hospital is a friendly and accessible hospital set in a 19-hectare parkland site on the edge of Bury St Edmunds. It has around 430 beds and serves a population of around 275,000 within an area of about 600 square miles. An acute hospital with an accident and emergency department, the West Suffolk Hospital aims to be the hospital local communities choose first, every time.
