Our Team is based in Cirencester, and we work closely with the GP practices in the Primary Care Network, alongside our Adult Social Care colleagues and voluntary organisations in the locality.

As you will be driving around the beautiful Cotswolds delivering our service, being a car driver is essential to the role. There might be opportunity for onsite accommodation at Cirencester Hospital - please get in touch for details., In line with the Government hospital discharge and community support policy, and the discharge to assess model, our Home First service is a time-limited assessment service for people in their own homes to facilitate rapid discharge from Acute and Community hospitals.
We also provide Reablement to patients in the community, to prevent hospital admission and to facilitate maximum independence, working on jointly agreed goals, promoting personalised care.

As a Physiotherapist you would be a key part of our team, providing assessment, goal planning and reviews, working alongside Occupational Therapists, Nurses, Reablement workers and rehabilitation assistants to enable our patients to gain their maximum level of independence in their own home. The caseload will be varied, ranging from rapid assessment to prevent hospital admission, Home First assessments to establish pathways of care following hospital admission, through to setting goals as part of a reablement pathway.
