It'san exciting time for our Urgent Community Response (UCR) Serviceas we continueto build on our successful model.UCRservices work with patients in their homes acrossBrighton & Hove and EastSussex communities to avoid hospital admissionsandsupport those being discharged home. Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver:
- A rapid response to people in the community who are experiencing acute episodes of ill-health.
- A supported discharge service tofacilitatetimelyhospital discharges.
- Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living. We are looking for aSenior Occupational Therapist to join us, so ifyou'rean ambitious, proactive, and experiencedOccupational Therapist looking to take a lead role in a team that providescutting-edge, excellent acute community care,we'dlove to hear from you! In this role, you will use your skills toundertake acute assessment,planningtreatment and intervention for patients with a variety of conditions,identifyingongoing needs and makingappropriateonwardreferrals to our partners. You will be part of theco-ordination of the multidisciplinary team through the allocation of visits. You will also support the clinical and operational leads in the supervision and development of colleagues and can develop your clinical and non-clinical capabilities across the four pillars of practice. We are the main provider of NHS community services across East and West Sussex. Our 6,000 staff serve a population of 1.3 million providing essential medical, nursing, and therapeutic care to adults and children.
