Full Job Description

We are looking for an experienced, motivated and enthusiastic Physiotherapist to operate in the Community Resource Team within Blaenau-Gwent. The individual will be working closely within the multi-disciplinary team, whilst working alongside colleagues within Health and Social Services. They will be expected to maintain a preventive mindset, operating closely with the Information, Advice and Assistance (IAA) Team., The team operates out of ViTCC, covering the whole of the Borough. As a Physiotherapist, you will provide clinical and professional guidance. You will be supporting and assessing individuals requiring a physiotherapy and reablement service as well as individuals who may require a period of assessment to identify their needs and outcomes. You will be an integral part of the wider multidisciplinary service comprising of Social Workers, Physiotherapists, Occupational Therapists, Rapid Response Team, Community Ward's, SALT and Reablement. You will be expected to develop and maintain excellent working relationships with a range of professionals and community organisations, in order to support individuals living in the local communities and in line with the Clinical Futures programme and place-based care., To provide a high standard of Physiotherapy within the Community Resource Team. Potentially contributing to Falls and remaining supportive of the development of students and rotational physiotherapists. Providing a preventative service and working closely with health and social colleagues as well as key working hospital discharges into the community setting. You will need to be a skilled physiotherapist who ideally possesses direct community experience and will be expected to carry a defined caseload of individuals requiring therapy support. You will be conversant with relevant legislation that informs the development of community care services locally and will be involved with any future proposed changes to services in Blaenau-Gwent.

To autonomously work alongside members of the multidisciplinary team ensuring best current, evidence based clinical management of patients. The post holder will work as an autonomous practitioner and will have relevant post registration experience for the role and will undertake clinical duties as required.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
