Norfolk and Norwich University Hospitals NHS Foundation Trust Therapy Service has a vision of developing and providing a high quality Home First Discharge service for our patients discharged home from the Acute Trust. The service will work in collaboration with the community & volunteer sector to provide safe and supported discharge followed up by a comprehensive person-centered holistic assessment, identifying recommendations to support regaining independence and improving health and wellbeing. This fantastic developing service offers you the unique opportunity to develop a service and deliver person-centered assessments supporting patients' individual needs and recovery. You can expect to work in partnership with patients and their families to get the best possible outcomes that can be shared and celebrated, identifying how the service meets the Government Hospital discharge and community support policy and operating model from October 2021. The Discharge to Assess Home First Therapy Team represents a new direction for NNUH and is in line with our commitment to provide the best care for every patient. You will receive supervision and annual appraisal; have access to peer support and encouragement to take an active role within your own continuing professional development.
