Full Job Description
An exciting opportunity has arisen for an nurse with specialist palliative care experience to work in our newly reopened Day Services. A new model for the service is being delivered incorporating outpatient clinics and patient and carer education sessions to encourage self-management. We are also keen to explore ways in which we can expand the service to reach those patients and carers who would normally not engage with our services. Meadow House provides a specialist palliative care service for the residents of the boroughs of Ealing and Hounslow. The Specialist Palliative Service comprises of a 15 bedded in-patient unit, day services and a large multidisciplinary community team. The successful candidate will work closely with the Day Services manager and other members of the multidisciplinary team to support patients and their carers in assessing and creating management plans for palliative care physical symptom management, social, financial and psychological issues.,
- Delivery of nursing care to all patients in the Day Services setting
- Supervises and coordinates patients, families and volunteers, providing direction and support in absence of the Day Services Manager/CNS.
- Effective discharge planning.
- Assist in meeting the requirements of the Clinical Governance programme pertaining to Day Services.
- Ensures the assessment, planning, implementation and evaluation of patient care in the Day Services setting.
- Acts as a resource and educator to staff, patients and their carers and pre-registered students in Day Services.
- Ensures effective communication within the Day Services both among staff and with patients and their relatives and carers.
- Deputises for the Day Services Manager/CNS as required.
- Represents the Day Services at meetings internally and externally in absence of the Day Services Manager/CNS
- Develops specialist skills and knowledge that are required to deliver palliative care in a Day Services setting, We promote the wellbeing and safety of all our patients and carers by acting promptly on concerns, communicating effectively and sharing information appropriately, according to the standards determined by our policy and procedures.
