Service Coordinator

A fulfilling and exciting opportunity available for a Service Coordinator to join our Association. The Service Coordinator will be based at our offices in Sheffield.

Purpose of the role
• The primary purpose of this project is to provide support to people with mental health support needs to live as independently as possible in their own home or temporary accommodation.
• The post holder is responsible for developing and delivering a person centred support package that is bespoke to suit the needs of each individual accessing the service.
• The support should assist service users in developing the skills required to build resilience to maintain their independence.
• You will be expected to contribute to the maintenance and development of the Association's values, culture and ethos.
  
  Salary up to £23,985.00 per annum for 37.5 hours per week,
