Full Job Description

We are currently looking for a hardworking, people focused person to join West Glamorgan Shared Lives Service. West Glamorgan Shared Lives is a newly establish regional support service that supports Shared Lives Carers across the Neath Port Talbot and Swansea area. Shared Lives is a scheme whereby people who are eligible for care and support are matched with suitable carers to live short term, long term or short periods of respite in the carers home. We need you to help us support the carer in their Shared Lives Carer role.   • Recruiting, assessing, monitoring and supporting Shared Lives Carers.
  • Managing new referrals to the service,
  • Assessing individuals who may wish to use the service and matching them with potential Shared Lives Carers.
  • You will be responsible for co-ordinating and managing a diverse caseload, to supervise and develop Shared Lives Carers in their role.
  • In addition, you will work with the individuals they support, their families, other professionals and a wide variety of stakeholders.
  • To work in line with the Adult Placement Services Regulation, Guidance and Local Authority Policies, and the National Minimum Standards, Recruiting, assessing, monitoring and supporting Shared Lives Carers.

    You will have experience of working with people in their own homes, disabled people, their family and others within their networks. Experience of managing a busy caseload and being able to organise your time is important. You will need to be a confident communicator and able to prioritise and adapt your working in a busy team.
  • Ideally you will have a City & Guilds Level 4 Adult Placement/Shared Lives (or be willing towards the qualification within the first two years of employment). This is an exciting opportunity to be part of a Regional Team Across the Neath Port Talbot and Swansea area.
