Full Job Description
This is a pivotal role within the Operations team to provide a confidential and comprehensive patient centred service regarding patient flow. To support delivery of the Key Performance targets.
The post holder works with all adult inpatient services to support identification and placement of and patients appropriately and timely, working to agreed protocols and procedures.
The post holder will be expected to work with the minimum of supervision and co-ordinate his/her activities with the members of the Team in order that an efficient service is provided.
The post holder will be expected to spend periods of time working under his/her own initiative. The post holder will carry out the duties in such a way as to make a direct and positive contribution to the organisation of the work and to assist the Team to maximise the most effective bed management service. It is essential that the post holder exercise initiative commensurate with the role and vital that confidentiality is maintained at all times., 1. Focus on patient flow through the emergency pathway
2. Sufficiently skilled in understanding the Emergency Department (ED) Clinical Quality Indicators including the criteria for 4 hour, 12 hour targets set nationally and support the specialist teams in ensuring flow is provided to achieve this
3. To support the allocation of beds in a proactive manner, considering patient safety at all times
4. Collate and record data daily relating to patient flow and performance in order to ensure the Trusts position is recorded both internally and externally as required
5. Interpret and convey information accurately and concisely and respond to changing priorities and conflicting demands in an effective manner and develop systems to improve effectiveness of the post
6. Ensure the nurse in charge, manager of the day and Clinical Site Manager is aware of the changing pressures within ED/MAU/SAU/MINORS and medical/specialised review requirements, 1. Responsibility for escalating delays to a patients journey across the organisation, to the relevant group
2. Actively assist in the organisation of urgent admissions and liaise with everyone involved in a professional and sensitive manner
3. Assist with the physical movement of patients around the organisation, if required, to support the delivery of good patient care and experience alongside delivery of the national standards of performance

5. Coordinate the workload of the Transfer Team to ensure flow and delivery of National Targets
6. To be responsible to ensure that the current "Real Time" information of bed status is crucial at all times of the day to ensure patient experience is not compromised
7. Responsible for ensuring that a comprehensive updated AM/PM handover is communicated to the night team of the sites capacity as well as any issues giving detailed information, with accurate future flow across the site
8. Provision of an effective internal and external communication system with agencies including PTS, IDT and private ambulance service
9. The role of Medical Bed Capacity Manager for all specialities is taken daily from 15:00 - 19:30 and across all weekends and Bank Holidays

10. To work on own initiative by accurately recording and seeking information when not directly provided, within the scope of the role
11. Prioritises and multitasks within the demands of the role
12. Collaborative working across all groups within the organisation
13. To be sufficiently skilled in the use of internal and external databases to ensure correct recording of information required for internal and National reporting
14. Efficiently use CRRS, EPR, EMS, NHS England Data collections and most Microsoft packages
15. Negotiating and influencing skills
16. Ability to problem solve and take a 'think outside the box' approach
