Full Job Description

To provide clinical support, consultation and advice, working freely within the 4 Tier model. To ensure the delivery of expert assessments and interventions and ensure care is delivered effectively and to a high standard. Ensuring that everybody involved in the case are involved including other professionals, outside agencies, carers and family. Ensure the delivery of consistent care, professional boundaries are maintained and based around evidence based in accordance with best practice. To ensure we are able to clearly identify risks and any safeguarding concerns and escalate accordingly. To ensure all child protection, adult safeguarding and Mental Capacity Act issues are dealt with according to policy., The post holder will be based within the Social Care Hub and will work as part of a centralised Social Work Team. Within the Social Care Hub, social workers have a specific focus upon the needs of adults of working age with mental health problems who have social care needs. They will lead on :

  • Care Act Assessments
  • Requests for Tribunal Reports for unknown cases from Wotton Lawn / Charlton Lane / Recovery Units and people who are from out of county
  • Reviews of individuals that Gloucestershire have placed out of county
  • Reviews of accommodation cases
  • Section 42 enquiries
  • Provide a multi-organisational crisis contingency and relapse planning function
  • Be the link for those that Gloucestershire has a Section 117 responsibility for, but reside outside Gloucestershire
  • Undertake the interface social care transition work for young people moving into adult services.
  • Act as Practice Educator for Social Work students and any ASYE social worker.
  • Arranging the provision or purchase of social care / health services to meet the assessed needs of individuals and their carers in more complex cases, whether within Gloucestershire or at a distance and to review such placements.
  • Enabling access for individuals to direct payments / personal health budgets.
  • Providing individuals and carers, in the community, with information and advice on appropriate sources of emotional, material or financial assistance, directly assessing, advocating and assisting in more complex cases.
  • Working in accordance with statutory and legal requirements offering specialist knowledge and consultation to the team on The Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 / 2007, Human Rights Act and a range of social care policy issues including appropriate adult and property protection, maintaining knowledge of and providing a bridge to local social care and County Council services and resources including those for child care and disabled adults / older people.
  • The social worker will maintain registration with Social Work England, holding to the associated values, in particular proactive implementation of anti-oppressive practice within a multi disciplinary setting.

    You will have experience of working with people with severe and enduring mental health problems including forensic experience and have Social Work Qualification.
