Covering Central Norfolk, our team of Social Workers, Assistant Practitioners and Occupational Therapists utilise the Care Act (2014) to assess and provide support to adults living in the community. There will be also opportunities to work within the Discharge to Assess Follow up team. With our integrated working, youll enjoy variety in your day and the opportunity to build strong working relationships with professionals across the county council, NHS, and other partners. We work under Living Well model of care to provide better outcomes by talking about what people can do, listening and connecting to enable early intervention in the community and avoid where possible admission to an acute setting.
