Caerphilly County Borough Council is seeking an experienced and dedicated Social Worker to join our 16 Plus Team. You will play a key role in supporting care-experienced young people, providing high-quality assessment, care planning, and intervention. • Provide high-quality assessment and care management for young people requiring ongoing intervention.\n
• Review care plans within statutory timescales, ensuring they align with the young person's needs and aspirations.\n
• Work in partnership with education, housing, health, and support services to promote positive outcomes.\n
• Offer guidance and practical support to young people, empowering them to achieve independence.\n
• Represent the service at court hearings, panel meetings, and multi-agency forums.\n
• Ensure effective multi-disciplinary collaboration with partner agencies.\n
• Contribute to service development initiatives to enhance outcomes for young people.\n
• Manage your caseload with supervision and support, ensuring accountability in your practice.\n
  
  \nQualifications: Degree in Social Work and registration with Social Care Wales.\n
• \nExperience: Experience working with care-experienced young people, safeguarding, and care planning.\n
• \nSkills: Strong assessment, advocacy, and communication skills.\n
• \nKnowledge: A sound understanding of legislation, policies, and best practices related to looked-after children and care leavers.\n
