Full Job Description

An exciting opportunity for a Band 5 Registered Nurse has arisen on our Discharge Lounge. The post holder must be registered with NMC UK registration.

We are looking for an enthusiastic nurse who can work dynamically and at a fast pace to ensure safe and effective care delivery through the Discharge Lounge. As a member of our team you will work closely with patients, their families and the multi-disciplinary team to ensure that high quality care is delivered and a positive experience for patients through the Discharge Lounge.

Main duties of the job

Using clinical knowledge/expertise,provide a high standard of skilled careto patients based on a full assessmentof the individuals needs in consultationwith the multi-professional team.

To devise and evaluate patient careplans ensuring that realistic goalsare set in consultation with thepatient, their family and themulti-professional team.

To ensure due regard is given tothe customs and spiritual beliefsof patients, carers and relatives.

To promote and role model effectivecommunication within the multi-professionalteam.

To act as a link nurse.

To identify the learning needs ofpatients, relatives and staff and,in consultation with the Sister/ TeamLeader, plan and implement programmes.

To ensure all documentation adheresto the Trust's documentationstandards.

To ensure evidence-based practice isdelivered within the clinical areathrough participating in research orfacilitating the application of researchfindings into practice.
