We are looking for a Care Liaison Advisor to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients. This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.   • Act as the vital link between discharge teams, local authorities, clients, and care providers, ensuring the delivery of high-quality services within set timescales, working towards a quick, safe and correct discharge plan for patients
  • Engage with families in a sensitive and empathetic manner, providing the support they need during the discharge process.
  • Work closely with care homes, care agencies, senior health and social care staff, patients, and their families.
  • Source both short-term and long-term care home packages, ensuring a quick, safe, and accurate discharge plan for patients.

    Previous customer service experience with excellent written and verbal communication skills
  • Previous administration experience with good attention to detail
  • The ability to organise and prioritise workload effectively
