Full Job Description
This role is intended to become an integral part of the PCNs multidisciplinary team, working alongside social prescribing link workers and health and wellbeing coaches to provide an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN. There may be a need to work remotely depending on the requirements of the role. Please note that the role of a care coordinator is not a clinical role Key Responsibilities: Dispensary Support: Assist with the accurate and efficient preparation, dispensing, and distribution of medications. Coordinate the management of repeat prescriptions, ensuring timely processing and effective communication with patients. Monitor stock levels, reorder medications, and manage supply chain logistics. Patient Coordination and Support: Provide personalised support to patients with medication-related queries, ensuring clear communication and understanding. Assist patients in understanding their prescriptions,
medication regimes, and any necessary adjustments. Engage in proactive follow-up with patients to ensure adherence and resolve any medication-related issuesCare Coordination and Communication: Collaborate with GPs, Pharmacists, nurses, and other members of the multidisciplinary team to coordinate patient care and enhance medication safety. Act as a point of contact for the dispensary, coordinating between the clinical team and patients to address medication needs. Liaise with community pharmacies and hospitals to facilitate seamless care transitions.Administration and Record-Keeping: Maintain accurate records of all dispensary activities, including dispensing logs, patient interactions, and stock levels. Use practice systems and software to log and manage patient information securely and in compliance with data protection.Quality Improvement and Patient Safety: identify and report any discrepancies or issues with prescriptions, medications, or patient records. Participate in audits,
quality improvement initiatives, and patient feedback activities to enhance dispensary services.Training and Development: Support the development of resources and guidance to help patients navigate dispensary services. Stay updated with best practices in medication management and support training for colleagues as needed.
