Full Job Description

The Transformation Manager role within the 0-25 Family Health Service will progress integrated pathways and systems working across County Durham through strong partnership working and co-production, using a data, intelligence and asset based approach. The successful candidate will be a proven change agent and role model who can demonstrate a visionary and innovative approach to care with a good understanding of the local and national health and social care agenda. They will be able to deliver service change and transformation withina complex and dynamic strategic and operational environment. The postholder will work closely with Public Health Integrated Commissioning and Key Partner organisations across County Durham. The Transformation Manager will be responsible for
providing the leadership to drive forward key integration and transformation projects acrossthe children's services.

Main duties of the job

Working closely with the Head of Public Health Nursing and Operations 0-19/25 the postholder will lead, plan and implement projects relating to the transformation and integration agenda, and implementation into the 0-25 Family Health Service model. The Transformation Manager will be part of the 0-25 Family Health Service Integrated Management Team, and will have line management responsibility and thematic lead area responsibilities at local and HDFT Children and Young People's Public Health Directorate levels. The Transformation Manager workplan will be regularly reviewed and updated in partnership with Public Health to address key priorities across County Durham to ensure Best Start in Life, reduce health inequalities and ensure continuous improvement. This includes but is not limited to: Family Hubs Breastfeeding Tobacco Dependency and exposure to second-handsmoke Healthy Weight Perinatal Mental Health Speech and Language and School Readiness Outcome measures Representation of the service
at strategic level across County Durham and regional forums. Governance: contribute to policy and process developments, learning lessons and continuous improvement processes. Utilise service user feedback to develop services.
