Transition Practitioner

The Transition Service will support young people and their family through their teenage years up to the age of 18. Building up their confidence and ability to manage the care and support they may need in adulthood. You will work closely with young people with life-limiting conditions, and their families, to assess, plan and co-ordinate the delivery of age appropriate services to support identified needs, including Advance Care Planning.

As a highly experienced nurse, social worker or allied health professional you will be a positive role-model for your team, offer expert advice and support, pro-actively developing your team's skills, knowledge and confidence.

Your role as Transition Practitioner will support staff, children and families at the Ty Gobaith hospice.

You will put the children and families at the heart of everything we do, you will have the opportunity and time to build meaningful relationships with children and their families, which makes this an incredibly fulfilling role. Hope House Children's Hospices have an opportunity for a Transition Practitioner to join our team based in Ty Gobaith, Conwy. You will join us on a part time 22.5 hours, permanent basis. In return, our Transition Practitioner will receive a competitive salary of £37,088 to £44,085 per annum F.T.E.
