Full Job Description

You will be a registered nurse with skills in palliative / end of life care, wound care, catheter care and urgent community 2hour response for patients and a strong desire to further develop your community nursing skills and experience., As part of the Urgent Community Response (UCR) Team you will be required:o To work within the Urgent Community Response Team to prevent unnecessaryadmission to hospital.o To assess and prescribe care pathways for patients in a crisis situation withlong term conditions, the frail & elderly, palliative and end of life care and/orrehabilitation needs, to achieve quality of life and independence wherepossible, with support from Senior colleagues within the wider UrgentCommunity Response Team.o To act as the patients advocate, facilitating choice and patient empowerment.o To provide evidence based clinical/therapeutic interventions based on "bestpractice".o To work within the Urgent Community Response Team to support earlydischarge from hospital.o To work with all health care professionals, and statutory/non statutory agenciesto provide a seamless, integrated service to our service users.o To triage referrals on a rotational basis for the Urgent Community ResponseTeam.

You will be required to work over a 7 day working week including weekends and will be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.

The post can be based at the new dedicated district nursing "Hive" building at the Norwich Community Hospital. You will be working closely with the other professionals with the urgent community response and virtual ward as well as link in with the planned community nursing teams. This dynamic and busy team require keen and motivated Nurses to provide assessment and quality interventions, in particular unplanned care to the population of patients restricted to receiving care in their homes.
