Full Job Description
The post holder will be able to assess, plan, implement and evaluate transition support for young people on the renal ward, renal out-patients clinic and dialysis unit in support of qualified staff.
To develop and deliver a comprehensive preparation programme for admission to the transition programme for a successful transfer to an adult renal hospital as well as prepare the young person for self-management of medical condition as an adult
The successful candidate will work closely with the renal transplant nursing and medical team. The successful candidate will also support young people in the haemodialysis unit and the other specialist renal modalities. Where deemed necessary the youth and transition workers will also support the attached renal ward. This role also has an element of play and support for our younger renal children and introduction to renal transition., Promote choice, wellbeing and the protection of all individuals, by being aware of the trust policies on child protection and safeguarding children and by following the correct procedures if you have any concerns.
To establish effective rapport with groups, individual children, their families and siblings.
Management of the transition clinics including planning, organising and delivering transition clinics alongside the adult hospitals. By assessing how many patients will attend each clinic, the complex needs of the young person that may involve other services, how many clinics each young person may require before transfer and what other support they will require through the transition programme.
Proactively participate in multidisciplinary meetings with the adult hospital pre- and post-clinics to plan the best tailored transition package of care for each young person. This will focus on how many clinics each young person attends, taking their needs and conditions into account.
Act as the young person's advocate with supporting them to gain confidence in talking to health professionals as well as taking control of their own medical condition and to live as good quality a life as possible by empowering the young person.
