Full Job Description

The role entails working with those affected by substance and alcohol misuse providing safe and effective interventions, harm reduction advice, and use of psycho social interventions to support service users to consider and achieve recovery.

The team consists of a well-established mix of professionals that work collaboratively to deliver individualised care. The role involves individualised clinical assessment, care planning, formulation, and collaborative working across the multi-agency team.

The applicant will be expected to demonstrate leadership within the team, providing supervision and supporting students.

The applicant will be expected to cover bases that fall within the Northumberland Recovery Partnership's geographical area which includes Blyth & Ashington, Physical health Nurse

The post holder will work within our physical health pathway within the multi-disciplinary team to provide care to those affected by substance use.

The role entails working with those affected by substance and alcohol misuse that may have a range of physical health needs alongside. The role will be to provide safe and effective interventions, harm reduction advice and use of psychosocial interventions to support service users to consider and achieve recovery.

The post holder will complete comprehensive physical health assessments in line with Trust standards and offer appropriate interventions, onward referrals and monitoring of health. Your role will be pivotal in addressing health inequalities within areas of deprivation, delivering essential care to those who's physical health is negatively impacted by substance and alcohol misuse.

Older Persons Nurse

The post holder will work within the new Older Person's pathway within the multi-disciplinary team to provide care to those affected by substance use.

There will be an expectation of an outreach support where appropriate to support the older person whether this be in their home setting, a community base, care home etc.

The role entails working with those affected by substance and alcohol misuse providing safe and effective interventions, harm reduction advice, and use of psychosocial interventions to support service users to consider and achieve recovery.
