Full Job Description
We are looking to recruit a Carer Support Worker to support Carers of individuals open to the 2 Community Mental Health Teams in Stockport. Referrals will come from within the team and the successful applicant will provide valuable links between the patients and their families. Additionally the post holder will liaise with wider carer networks and be involved in the Triangle of Care Meetings. The work will involve working with carers individually or in groups. The post holder will take referrals from within the CMHT and will support and advocate for carers to ensure their voice is heard when care to an individual is planned. As a member of the CMHT Staff the post holder will attend meetings and act as the Carer Champion for the team. There will be opportunities for linking with other Carer support services from within the trust and wider Stockport Community. As a Carer Support Worker you will be responsible for supporting the Trusts Carer Strategy/ Triangle of Care within CMHT ensuring that our Carer's needs are met through the delivery of high-quality support. You will also ensure that our Carers are fully informed of opportunities to become involved with the Trust and the work it does, supporting the high standard of care we deliver to our service users. Work closely with clinical teams, offering support to carers in their own right. Be a point of contact for Carers to discuss their own needs. Act as a signpost for Carers to access support in their local area Liaise with the Carer and clinical team supporting visits. Support the development of and maintain a Carers database as necessary. Support the Trust in recognising the importance of Carers being involved in the care To ensure that the role of unpaid carers are recognised, supported and valued.
- To promote the Triangle of Care (TOC) standards and principles, and other carer specific initiatives to services and carers.
- To ensure that carers are well informed, supported with their individual health and well-being needs in relation to their caring role and involved in decision making about the care of their loved one. AFC 8/11/21 JME-290-21 Carer Support Worker Band 3
- To provide carers with information and advice about services both within and external to the organisation, that contributes to their caring role and increases their knowledge regarding care and treatment processes.
- To work collaboratively with carers and clinical teams to ensure that carers feel involved and supported in the care and treatment of their loved one in accordance with Service User's consent, Trust Strategic Priorities and Carers Engagement Procedures.
- To have knowledge in the identification and support required for young carers.
- To signpost and facilitate carers access to a range of support services.
- Provide 1:1 support to carers and access to information, advice and activities that help maintain and improve carer health and wellbeing, tailored to meet their individual needs.
- To facilitate the delivery of carer support in a group setting where required.
- To support carers attending clinical team reviews, ward rounds and CPA meetings.
- To support clinical teams working with families, friends and parents.
- The post holder will be able to respond flexibly to the needs of carers.
- The post holder will take an active role in the development of the service approach to supporting carer involvement and engagement and promote carers needs both internally and externally.and treatment of their loved one.
