Full Job Description
To support KCHFT health visiting teams deliver the Healthy Child Programme, improving outcomes for individuals, families and the community. Utilising a family focused public health approach and delivering intensive programmes for the most complex children and families. Manage a clinical caseload and work as part of the Universal Services Health Visiting team, in line with KCHFT Health Visiting commissioned service. Working in partnership with a variety of statutory and non-statutory agencies to develop plans to improve health outcomes for individuals, families, groups and the wider community. To contribute to the Kent Safeguarding agenda through common assessment, child in need and child protection participation., Band 6 Health Care professionals (HCP) work closely with the people who use our services, families, friends and carers and play an important contribution in how people experience our services. Band 6 HCPs are expected to be kind, responsive and professional, contributing to the quality of theservices we provide by:
- Keeping the people who use our services safe through the use of sound clinical skills and effective risk assessments
- Ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures
- Ensuring the people that use our services have a good experience by respecting, empowering and working in partnership with them.
