Full Job Description

Berkeley Vale Primary Care Network (PCN) is seeking a passionate and experienced Frailty and Dementia Lead Nurse to join our multidisciplinary team. This is an exciting opportunity for a dynamic nurse with expertise in dementia and frailty to take a leadership role in shaping and delivering high-quality, person-centred care across our network. You will work autonomously while collaborating closely with colleagues across the PCN and wider health and social care system. Your role will include service development, education, and clinical leadership, with a strong focus on enhancing the quality of care for older people and those living with dementia. Main Duties of the Job Act as a senior nurse for dementia and frailty within the PCN, providing specialist assessments, advice, and management plans. Lead the development of multidisciplinary frailty and dementia services across the PCN. Support the proactive identification and management of patients with frailty. Work in collaboration with
the PCN and other stakeholders to develop and improve frailty services. Provide training and education to staff on dementia and frailty to ensure they maintain up-to-date knowledge and competencies. Work in partnership with GPs, social care, and voluntary sector providers to support seamless, holistic care. Promote best practice, evidence-based care and contribute to continuous service improvement.
