Full Job Description
Working as part of the interdisciplinary team, the ACP will assess, plan, implement, and evaluate care for patients with complex needs, helping them to manage symptoms and improve quality of life.
To support the development of an evidence-based practice within the hospice and wider community, in collaboration with healthcare professionals and external partners.
To deliver care in line with the highest professional standards, adhering to the competencies and expectations outlined in the Advanced Clinical Practice in England, and the four pillars of advanced practice.
To complement the medical team and will be expected to contribute to the out of hours 1st on call medical rota.
Main duties of the job
Conduct comprehensive physical and psychological assessments of patients, developing personalised care plans to address their palliative and end-of-life care needs.
Utilise advanced clinical reasoning to diagnose complex conditions and initiate appropriate treatments.
Perform advanced interventions, including prescribing medications, managing pain, and implementing palliative care measures within the scope of practice.
Work collaboratively with the patient, family, and interdisciplinary team to ensure care plans reflect patient goals and are updated regularly.
