Full Job Description
Collaborate with multidisciplinary teams to optimize patient care and ensure effective medication use.
Obtain accurate medication histories, assess patients' own medicines, and support the transcription and supply of discharge medication.
Counsel patients on medication use, providing advice on benefits and side effects.
Facilitate audits, promote adherence to Trust and national clinical guidelines, and support quality improvement initiatives.
Provide training for pharmacy staff, nurses, and midwives regarding medicines management.

Responsibilities
IN THIS ROLE, YOU WILL:
Manage medicines at ward level, including stock control, non-stock item requests, and one-stop dispensing.
Support clinical pharmacists in reviewing prescriptions and ensuring the accuracy of dispensing.
Actively participate in discharge planning by coordinating medication supply and liaising with GPs and community pharmacies.
Assist with ward stock reviews and audits to promote safe, effective, and efficient medication use.
Act as a role model and mentor for junior staff, contributing to training and development programs.
Uphold the Trust's standards for clinical pharmacy, participating in risk management activities and ensuring compliance with all statutory and professional requirements.
Participate in a rotational shift pattern, including late nights, weekends, and bank holidays, to support a seven-day pharmacy service.
Main Responsibilities and Working for Your Organisation, Promote patient safety by ensuring the quality, accuracy, and timeliness of medication-related processes.
Support compliance with the Trust's governance and improvement practices.
Contribute to the creation and development of medicines management policies, procedures, and standards.
Collaborate with the wider healthcare team to maintain a patient-centered focus in all aspects of care.
This position is part-time, with a total of 22.5 hours per week. The current schedule is Monday to Wednesday, 8:30 AM to 4:30 PM; however, working hours can be discussed during the interview process to accommodate flexibility.
The role involves participating in the late-night service on a rotational basis, with operations extending until 7:30 PM. Additionally, the successful candidate will be expected to contribute to weekend and bank holiday shifts, as part of a rotation, to support the Trust's commitment to providing a seamless seven-day service
