Job Overview

INTERACT (Interact Responsive Anticipatory Care Taskforce) provides both proactive and reactive care and support for adults (18+) whose health and well-being are adversely affected by their housing situation or home environment and are experiencing one or more of the following; frailty, multi-morbidity or complex needs. You will play an integral role by supporting individuals and their families in accessing support services within their local community.

Objectives for this role:

· Provide detailed information, advice and guidance using motivational interview techniques, that will be delivered in person, by telephone and email.

· Work closely with clients to develop a Personalised Care and Support Plan (PCSP) understanding their needs, the desired outcome and the necessary interventions to provide support.

· Signpost and make appropriate referrals to voluntary sector services, community initiatives, and relevant public sector services to achieve agreed actions and outcomes.

· Offer short-term support to clients, fostering independence, confidence, and skill development

· Be part of a multidisciplinary team focused on supporting individuals whose housing situation impacts their health and well-being positively, providing practical and holistic assistance.

What we’re looking for:

· Clear communicator and ability to engage with individuals from diverse backgrounds.

· Ability to work collaboratively in a multidisciplinary team environment.

· Willingness to adapt to changing organisational needs and priorities.

· Knowledge of voluntary and community services in Norfolk and how to support people to access them

· Driver’s Licence and Car is essential.

A full training program will be given. This role will be subject to a DBS check
