Full Job Description

Would you describe yourself as dynamic, innovative, and professional? Are you looking for a new challenge supporting Children, Young people and adults aged 18-25? Would you be interested in being a part of an exciting National initiative? We are excited to reveal that we are looking for Dynamic Support Navigators (band 7) to join our "Keyworking Programme" at our base in Erlegh House, Reading. The aim of the "Keyworking Programme" is to trial a new way of working to support children and young people with a learning disability and/or autism, who are at risk of admission to a mental health hospital. As well as supporting children and young people you will also work with their families to make sure they are fully involved in their plans and they have the support they need at the right time, in a co-ordinated way. These are not clinical roles as such but strive to actively consult, navigate, and guide the teams around the service user to bring about interventions that are strengths-focused, evidence-based and tailored to meet the needs of the child/young person and their family/carer. The working hours for these roles are Monday to Friday 9am to 5pm and we are open to both full and part time applicants.Main duties of the job

  • Identify and engage children, young people and adults aged 18-25 who are at the highest risk of admission to a mental health hospital. This also includes young people who may have recently been discharged from a Tier 4 setting and are working to prevent a "revolving door" of readmission.
  • Work with CYP/adults aged 18-25 and their families, who are at the highest risk of admission to a mental health hospital to develop a personalised care programme plan that encompasses all aspects of their care. This will involve developing a flexible plan of work around the CYP/adults aged 18-25 and their family's needs and identified outcomes.
  • Communicate clearly with CYP/adults aged 18-25 and their familyto help them understand and navigate the system. For example: advising how to access legal and medical information.
  • Represent and champion the CYP/adults aged 18-25 and theirin discussions around provision of care as required, communicating compassionately and confidently with them and the range of professionals involved in their care.
  • Troubleshoot on behalf of the child/young person/adults aged 18-25 and their family where required to ensure the necessary support is put in place to enable them to remain within, or be discharged into, the community.
  • Promote joined-up, consistent care that meets their needs and rights. This may involve constructively challenging the 'status quo', bureaucratic decision making and decision making in silos.
