Full Job Description
The NA will work with a multi professional team contributing to ongoing assessment, providing & monitoring care in line with service pathways & as agreed with your supervising clinician(s).
You will monitor the condition & health needs of people on a continual basis in partnership with the multi-professional team.
The NA has a breadth of knowledge providing holistic & person-centred care & support for people in a variety of settings. You will work independently under the leadership of Registered Nurses, working within the sphere of nursing & care.
You will have ability to communicate effectively, with sensitivity & compassion.
We are committed to delivering high standards of multi-disciplinary patient focused care; the post holder would be an integral part of this & will be essential to improving service delivery.

Undertake comprehensive health care needs assessment, ensuring they are planned, implemented & reassess as appropriate for a range of health needs including wound care, medication support, insulin, continence, end of life care.
Have opportunity to learn new skills, giving you confidence in many aspects of complex health care.
Collaborate with other health care professionals, delivering high standards of effective care.
Be willing to learn & progress your knowledge.
Be committed to teamwork.
Have a positive attitude with a focus on team learning/support.
Have the skills to work across both our planned nursing & crisis pathways. Our aim is to support patients to remain well & independent in their own homes/place of residence & reduce the need for acute hospital admission.
Be supported with relevant training to gain necessary skills across both pathways.
