Full Job Description

Core business hours are 8.30 to 5 - Monday to Friday. A typical working week could include: supporting families with children who have complex needs; managing health needs assessments and attending review meetings for children looked after; transfer into area visits; triaging A&E attendances and offering follow up support to families; providing medical conditions training to schools and on a 1:1 basis with parents; offering short term packages of care to families/young people requiring specific support; signposting to other key services and completing referrals as appropriate. These skills and experiences will support your journey to be a qualified SCPHN alongside developmental shadowing opportunities, bespoke supervision and taking on lead roles to enhance care provision. Are you a registered nurse with an aspiration for working in the community to improve outcomes for children, young people and families? If so Leeds Community Healthcare has an unique and exciting development opportunity for you to join our 0-19 Service, initially as a Staff Nurse onto our development programme. This will support you to acquire public health skills and experience as you then move onto the Specialist Community Public Health Nursing (SCPHN) Programme offered at Leeds Beckett University and on successful completion of the course, return to the Service as either a Band 6 health visitor or school nurse.
