The Department of Ageing and Health is seeking to recruit 2 x substantive consultants to join our dynamic, enthusiastic and forward-thinking team of highly motivated and dedicated geriatricians. Ageing & Health has a number of innovative and award-winning services across a number of areas including geriatrician liaison (POPS, GOLD, COPS), acute frailty and in the community. The advertised posts will support the delivery of these services, in particular delivery of Perioperative medicine for Older People undergoing Surgery (POPS) in thoracic surgery, front door service for frail older adults on the emergency floor, specialist Older Persons' Unit wards, in General Internal Medicine, as well as in the community.
The post holders will either contribute to an elderly medicine on call rota which includes on site weekend working in our Acute Older Persons Unit in the Emergency floor, or to the Acute General Medicine rota which includes on-site weekend and evening working., The Department of Ageing and Health is a dynamic department right at the centre of Acute Medical, Surgical and Specialist Patient Services at Guy's and St Thomas' NHS Foundation Trust. It has researched and developed many new service delivery models in Geriatric medicine, and as a consequence is an ideal place to work as a geriatrician. The department provides specialist services for older people within the Acute Trust and throughout Lambeth and Southwark supporting community services. In the Acute Trust services include its own inpatient unit at St Thomas' Hospital (Older persons unit (OPU)), participation in General medicine (acute emergency floor and general wards) and input across multiple areas by our liaison teams including POPS (surgery and orthopaedics) and GOLD (oncology). It has its own specialist outpatients based at Guys (OPAU) and a newly established front door Acute-OPU within the emergency floor. Most geriatricians also contribute to work

The advertised posts will support the delivery of these services, in particular
- delivery of Perioperative medicine for Older People undergoing Surgery (POPS) in thoracic surgery
- In the acute medical pathway (including the front door service on the emergency floor, in the medical outreach service, and on our specialist wards for older people)
- specialist Older Persons' Unit wards & in General Internal Medicine
- clinic work (POPS, CGA, GOLD)
- community multidisciplinary working with our primary care colleagues (including work within rehabilitation units, care homes and with the GSTT@home service)
This is an exciting time to join the department. The acute medical pathway is undergoing radical change to ensure a consultant led service is delivered 7 days a week during extended day hours and 365 days a year. The department of Ageing and Health contributes to this by providing a front door service 7 days a week within our Acute Older Persons Unit (AOPU) which is co-located with the emergency floor. Geriatricians also participate in the acute GIM emergency floor rota and lead firms on general medicine wards and in the older persons' unit.
The multidisciplinary POPS team provide preoperative assessment and optimisation for high risk older patients undergoing elective and emergency surgery, and follow the patient through the surgical admission, coordinating and delivering medical care, rehabilitation and discharge planning. The team is fully embedded at GSTT, with established pathways of care, close collaboration with anaesthetic and surgical departments and has a local, national and international reputation for providing quality care to complex, older surgical patients. The team provides formalised training programmes in perioperative medicine for foundation year doctors (FY1 and 2), specialist registrars and allied health professionals.
The acute trust is merged with therapies, community services and palliative care. Ageing & Health has increasing footprint across the community including the Hospital@Home team, bed-based and community rehabilitation, care homes and multidisciplinary services to support developing integrated neighbourhood teams.
The successful candidate is not expected to have received formal subspecialty training in perioperative medicine for older people or other subspecialty discipline. All posts include working alongside multidisciplinary teams to develop and deliver patient centred services. In all roles, the successful candidates will be expected to be intellectually flexible and able to look beyond existing structures, ways of working and traditional care boundaries in order to produce more effective and innovative service delivery. The post-holder will be expected to take an active role in in the teaching programmes of the department.
The successful candidate will work within the wider Department of Ageing and Health at GSTT, staffed by 26 consultants, with various subspecialty interests. The unit has a strong reputation for innovative service delivery, training in geriatric medicine and research. All consultants in the department are expected to share equally in out-of-hours rotas (either the acute GIM emergency floor rota or the acute frailty rota). The successful candidate may be able to pursue other subspecialty upon local negotiation.
