Full Job Description

RightCare Barnsley Patient Flow Coordination Team is expanding, we are looking for new people to join our growing team.

Rightcare provides a single point of contact for health and social Care staff across Barnsley for patients requiring hospital or community services. RightCare also takes referrals from Barnsley Hospital wards and departments for patients requiring discharge support and works closely with the Hospital Discharge and Patient Flow Team. RightCare also coordinates admissions and discharges for the Intermediate Care Service.

The aim of the service is to ensure that the right care is provided, at the right time and in the right place. You will work closely with health, social care teams and professionals from primary, secondary and community care supporting the RightCare nursing and coordination staff access acute care or other more suitable alternatives to hospital admission that will meet the needs of the service user., As a Patient Flow coordinator, you will be based in the RightCare call centre and will deal with referrals taken electronically and over the telephone. You will be part of a wider team including Nurses, administrative officers, Social Care and Intermediate Care staff. You will send referrals to a range of health and social care providers, including Care Homes, you will follow up the referral to confirm service provision and let the relevant teams know.

We are looking for an individual who is keen to develop their knowledge and skills in relation to acute and community services, who have drive and passion and are committed to ensuring that the aims of the RightCare service is delivered for the benefit of patients and public.

For this role it is essential that you are a confident, organised and have significant experience in the acute and/or community care setting. You will have excellent interpersonal, communication, decision making and influencing skill in order to ensure the highest standards of care are achieved. The role is to provide comprehensive and pro-active coordination and support, ensuring that all processes and tasks are undertaken efficiently, effectively and to a high standard, contributing to the smooth running of the service and its overall effectiveness
