Are you looking for a change within the pharmacy world? Want to work with a growing Medicines Management Team? Would you like to develop your skills as a clinical pharmacist and help patients' with their medicines? If yes, this could be the post for you: Central London Community Healthcare NHS Trust is large community trust working with its local PCNs, looking to recruit a dynamic and innovative pharmacist to join the Medicines Management Team for an exciting post as 8a CLCH North Hammersmith & Fulham Primary Care Network Pharmacist (depending on experience, fixed-term 12months). They will work alongside our well established CLCH pharmacy team in delivering the pharmacy PCN service. Key responsibilities include: Providing specialist expertise in the use of medicines, conducting structured medication reviews for patients with inappropriate polypharmacy and overprescribing, developing and implementing long term condition prescribing clinics and acting as a source of pharmaceutical expertise to various health and social care professionals e.g. GPs, district nurses, carers, families, community pharmacists and care coordinators. Trust wide Medicines Management Service Provision including policy development, implementation of patient safety initiatives e.g. drug alerts, patient safety alerts, training for health professionals in the community across the Trust and PCN., The post will involve working in GP practices within Hammersmith and Fulham North PCN, including lone-working and travelling between sites.
The post holder must have relevant post registration experience in hospital pharmacy, community pharmacy or GP practices and a postgraduate qualification to at least diploma level or working towards or completed the CPPE Primary Care Pharmacy Education Pathway as well as independent prescribing qualification or currently working towards completion of this. A sound knowledge of prescribing and medicines management issues faced in primary care is essential with excellent decision-making and problem solving skills. In addition a wide exposure to and well-rounded knowledge of various clinical conditions and their management. Additionally, compassion, flexibility, excellent communication and time management skills are essential and the ability to build relationships with various health and social care professionals e.g. GPs, district nurses, carers, family, community pharmacists and care coordinators.
