Full Job Description

This is a 12 month fixed term part time (27.5 hours) post working across Monday to Friday covering a secondment until 1st December 2025. Applicants MUST have activity co-ordinator experience in a reablement/rehabilitation setting, ideally in the NHS. Level 2 Health & Social Care is essential. An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks. This vacancy may close early if sufficient applications are received.,

  • As part of the multidisciplinary team, the post holder will help deliver and encourage clients to participate in socially interactive activities;
  • The post holder will actively lead on promoting patients remaining active during illness and will engage with the Healthcare Assistants on the unit to ensure activities are available at all times;
  • The post holder will identify a varied and interesting activity programme for service users;
  • Document the engagement in activities by the service users and record in service user records;
  • The post holder will ensure the activities available on the unit are appropriate for the client group;
  • Support alongside the unit team to ensure patient fundamental care needs are met and dignity is maintained;
  • The post holder will network with appropriate and relevant partners and agencies to promote healthy relationships between Community and Acute services;
  • The post holder will actively lead publicising the activities and monthly campaigns on the unit, to ensure the profile of the unit is heightened;
  • The post holder will contribute to maintaining a safe and effective working environment, being assigned duties from the Nursing/Therapy Leads and the Team Leader

  • With support from the ward leadership / multidisciplinary team, contribute to proving a range of social interaction and well-being activities, e.g activity groups, and on an individual basis, with minimal supervision.
  • Under guidance from the shift co-ordinator / unit manager / therapy lead, prioritise workload according to client need and unit needs.
  • In collaboration with the client group and unit manager / therapy lead, maintain and develop the activities timetable for the unit and for each client on an individual recovery focused basis.
  • Work alongside colleagues e.g. Occupational Therapy, Physiotherapy and Nursing staff to deliver a range of activities that are recovery focused and suit the individual needs of each client.
  • Contribute to the regular updating of individual activity timetables for clients which offer a range of activities on an individual and group basis dependant on the client's needs. This can be subject to regular change.
  • Engage with clients both within and out with the ward environment to ensure diversity in the activities being offered and maximise recovery potential.
  • Document all engagement with clients in their case notes to ensure the clinical team is kept up to date with client's participation in activities. Verbally hand over this information to the Therapeutic and nursing team on a shift by shift basis as appropriate.
  • Communicate with the multidisciplinary team regularly regarding client's participation or changes in activity levels.
  • The post holder will support learners in the workplace by providing an opportunity for shadowing with the activity co-coordinator to observe activities being planned and delivered.
  • Ensure the provision of timely fundamental patient care by working alongside the ward team to support patient with activities of daily living.
  • Actively engage with the rest of the ward team to ensure delivery of activities and diversional therapies outside of Activity Co-ordinator hours of work.
  • The post holder may be required to exert frequent moderate physical effort for several short periods during a shift e.g. moving patients over 15 kilos with mechanical aids.
  • You have a personal responsibility to support your department/ward/clinic in reducing hospital acquired infection.
  • You must comply with the Trust's policies on infection, prevention and control and maintain your competency to effectively discharge your responsibilities.

  • Planning and Organisational Duties

  • Communications and Key Working Relationships
  • To communicate with the multidisciplinary team regularly regarding patients' participation or changes in activity levels.
  • To build and maintain therapeutic relationships with patients and their families or carers.
  • Actively engage with the rest of the ward team to ensure delivery of activities and diversional therapies outside of activity coordinator hours of work.
  • Responsibility for Human Resources
  • To assess, identify and evaluate own knowledge and practice needs in relation to knowledge and skills required to meet the demands of the job.
  • To understand their own role and scope, identify own development needs and take responsibility for their continuing professional development and performance whilst maintaining a personal development plan.
  • To make effective use of appropriate learning opportunities for themselves and other and apply learning to practice.
  • To understand annual mandatory training updates and other relevant courses in line with Trust and local policies.
  • To act as a role model and support professional development of all healthcare assistants, apprentices and students working in the unit team.
  • To act as a mentor and assessor to assigned learners and to teach and support them through their competency assessment as required.
  • Responsibility for Health & Safety
  • Compliance with the Health & Safety at Work Act 1974 - the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
