Form professional relationships with patients to enable them to achieve the outcomes required within the care plan in a way that respects their views, autonomy and culture. Contribute to the assessment of individual patient needs as requested and directed by the responsible multi-disciplinary team professional. Support and implement individual care plans as devised by qualified members of the multi-disciplinary team. To support and assist in the delivery of Physiotherapy, Occupational Therapy and Complementary Therapy activities where relevant and appropriate to the overall plan ensuring an holistic approach. Take a lead role in supporting patients to participate in appropriate person centred therapeutic activities including undertaking the role of escorting patients in a full range of ward based and where relevant community based activities as agreed / planned by the multi-disciplinary care team or named nurse. To perform a range of supporting administrative tasks where required. To report all complaints and untoward occurrences to a qualified nurse. Contribute to effective communication with patients (including non-verbal communication methods), their relatives, friends, staff and other professionals to ensure the continuing of quality and safety in the provision of care., + In the event of exceptionally high levels of response, we reserve the right to close the post before the date stated above in order to prevent the number of applications received being unmanageable. - Priority will be given to NHS staff across the region who are at risk of redundancy, where the vacancy is at the same band or one below their current band and candidates meet the essential criteria for this post.
- All employees new to the Trust will be subject to a 6 month probationary period.
- All clinical staff bands 2-4 that are new to the Trust will be required to complete a Care Certificate.
