Contribute to the assessment of individual patient needs as requested and directed by the responsible multi-disciplinary team professional. Support and implement individual care plans as devised by qualified members of the multi-disciplinary team. To support and assist in the delivery of Physiotherapy, Occupational Therapy and Complementary Therapy activities where relevant and appropriate to the overall plan ensuring an holistic approach. Take a lead role in supporting patients to participate in appropriate person centred therapeutic activities including undertaking the role of escorting patients in a full range of ward based and where relevant community based activities as agreed / planned by the multi-disciplinary care team or named nurse. To perform a range of supporting administrative tasks where required. To report all complaints and untoward occurrences to a qualified nurse. • All clinical staff bands 2-4 that are new to the Trust will be required to complete a Care Certificate.
