Full Job Description
To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users. To act as the liaison practitioner for East CMHT with psychiatric acute services, including inpatient wards and crisis resolution and home treatment teams. To act as lead practitioner to an agreed caseload of service users with severe and enduring mental illness. To undertake ongoing assessment, treatment and evaluation for service users known to the service. To support the delivery of therapeutic interventions either individually or within a group setting. Support the transformation of the team to deliver local evidence-based care.,
- To lead the clinical management of a designated group of service users. To gather weekly clinical information from relevant inpatient acute services and report back to the team MDT. Attend Southampton Crisis team MDT to support in the review of East CMHT patients / service users and screen as appropriate, the suitability for CMHT support. Support Southampton acute pathway team in identifying barriers to discharge from psychiatric wards. To allocate work to junior colleagues, reviewing progress and agreed outcomes. To co-ordinate and monitor the delivery of person centred healthcare to a designated group of service users, using complex clinical decision making where appropriate. To work as a member of the multi disciplinary team in assessing and planning care and responding to changes in service users' health.
