Full Job Description

The post holder will be responsible for working within an evolving Medical SDEC service with the objectives of supporting delivery of Same Day Emergency Care to patients referred from both Primary Care and the Emergency Department., For a full list of main duties please refer to the Job Description.
· Provide clinical review and management of patients referred to the Acute Medicine service deemed suitable for Same Day Emergency Care
· Ensure handover of care documentation is completed for patients assessed in Medical SDEC including robust arrangements for follow up and outstanding investigations
· Work as a member of the Acute Medicine team, assessing and treating patients independently, seeking support as required from the supervising Consultant Acute Physician
· Provide competency appropriate clinical leadership for resident clinician and ACP's working in Same Day Emergency Care
· Provide advice and support to clinical staff referring patients to the Medical SDEC Unit
· Work according to local protocols and national guidelines, promoting safe standards of care and services, The Medical Same Day Emergency Care Unit on Gate 36 (consisting of a 2 triage rooms 5 consulting rooms, a procedure room and a treatment/observation bay). The Medical SDEC unit's ambition is to manage approx. 25-30% of these attendances as Same Day Emergency Care (SDEC) and up to 50% with a length of stay less than 48 hours.
The post holder will be responsible for working within an evolving Medical SDEC service with the objectives of supporting the delivery of Same Day Emergency Care to patients referred from both Primary Care and the Emergency Department. The post holder will be well-placed to support the transition of patients from Primary Care into Secondary Care and back again.
The post holder would be based predominantly in Medical SDEC Unit working as part of the wider Medical SDEC team, supported by a Consultant Acute Physician. This post will be one of several contributing to clinical care during Medical SDEC opening times. The post holder will also be expected to use their knowledge, experience, and expertise to inform and support the development of new elements of the Acute Medicine service pertinent to admission avoidance and Medical SDEC.
