The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable., An exciting opportunity has arisen for an enthusiastic and highly organised individual to join our Long Covid / Community Complex Conditions Service. The post is based in Bryn Y Neuadd Hospital and will provide effective and efficient running of the Long Covid / Community Complex Conditions Pathway by way of processing referrals into the service in a timely manner, being first point of contact to patients and providing the team with general administrative support. The role involves being an integral member of the Admin Team working under direction from the lead co-ordinator co-ordinating and administering the pathway and processing patient correspondence., Main duties include:
Help and support all members of the team by having a flexible approach to both workload and working pattern through management of own workload and providing advice and mentoring of new members of staff.
Process referrals forms to ensure patients are put on the Long Covid/Complex Conditions waiting list using the hospital computer system (Therapy Manager).
Check patient demographic on receipt of referral ensuring patient fulfils the criteria for the Long Covid service .
Inform Line Manager of any anticipated problems related to patients being seen and treated within their agreed waiting time. Arrange, book and contact patients to fill clinics working to patient booking rules.
Maintain a database to monitor patient progress with the Long Covid/Complex Conditions Service, recording of referral information and relevant patient feedback information
To ensure appropriate scheduling of clinics and appointments are made within a specific timescale alerting Lead Coordinator and Management Team of any lack of capacity and seek advice for a solution
Typing of clinic letters using EPRO system.
Print confirmation of appointment letters and dispatch to patients.
