Full Job Description

NHS Greater Manchester Integrated Care is looking for motivated administrative staff to join their NHS Funded Care Team (FCT) in Bolton. We commission safe, effective, caring and high quality health services mainly in the community, but also provide support to the Acute Trust and Mental Health Services.
You will be an administrative officer with experience and interest in supporting nursing staff. You will have experience and skills in providing administrative support to a busy clinical team.
You will be highly motivated, have the ability to work independently, but also as part of a team. You will have good IT, organisational and interpersonal skills and give attention to detail.
The post holder will be expected to liaise with clients, families, care providers and other professionals within the health and social care sectors. The post holder will also be required to support the clinical team members and case managers of complex Continuing Health Care patients and their packages of care, making the role varied from day to day.
The successful candidate will need to be able to demonstrate the ability to provide administrative support to a busy clinical team, have good written and verbal communication skills. They must have the ability to work in stressful situations and be able to prioritise and organise their own workloads to meet deadlines., Communication - To liaise with patients, nursing homes and outside agencies where necessary and to act as first point of contact for all queries concerning funding for the NHS Funded Care from patients, carers and outside agencies, ensuring appropriate sensitivity as required
Analytical and Judgement Skills - support administrative process and systems recording data appropriately
Planning and Organising - providing support to the Funded Care Team, attending meeting as necessary and to liaise with other professionals and members of the public as required.
Responsibilities for physical and financial resources - to ensure the correct updated personal/organisational and financial data is held for patient package in Broadcare. Good keyboard skills and knowledge of office IT applications, enabling data and reports to be processed in a timely manner.
The post holder is expected to be able to use their initiative although will work under the supervision of a Senior FCT Staff
Partnership Working/Collaboration - The post holder is expected to be able to work in collaboration with other NHS GM Integrated Care colleagues including Finance and Contracting Team and to work closely with staff at other NHS organisations and at Nursing Homes.
As a representative of the NHS Funded Care Team you will be expected to promote the team's vision, and mission and to uphold GM Integrated Care values in all day to day activities and delivery of services.

Working for our organisation
Bolton Funded Care Team comprises of a team of nurses and administrators. The team is responsible for ongoing case management for individuals with complex health/social care needs that are eligible for Continuing Healthcare funding.
You will be working within a professional and motivated team providing support to the nursing staff and managers. As a member of the Funded Care Team you will be employed by the wider organisation - NHS Greater Manchester Integrated Care. The team is dedicated to promoting individual development and will provide training and support within the job role as appropriate.
The role is office based in Lever Chamber in Bolton town centre, currently it does incorporate some home working hours (hybrid working) which may be subject to change in the future., To provide full administration support in Bolton NHS Funded Care team.
To administer a full range of clerical and administration services to the Bolton NHS Funded Care team.
Key Working Relationships
The post holder will be required to work with other NHS organisations including:
+ Acute Trusts, NHS Greater Manchester (GM) Integrated Care and Intermediate Care Centres
+ District Nurses
+ Nursing and Residential Home Managers, Clinical Leads and administrators
+ GP Surgery nurses and practice managers
+ Local Authority staff (including Social Workers)
+ Members of the public and patients.
The post holder will also be expected to supervise the communication between the FCT admin team and these other agencies and organisations.
Communication
+ To liaise with patients, nursing homes and outside agencies where necessary and to act as first point of contact for all queries concerning funding for the Continuing Care NHS Funded Care from patients, carers and outside agencies, ensuring appropriate sensitivity as required
+ To answer telephone, e-mail or personal enquiries relating to Continuing Healthcare, retrospective and further reviews
+ To act as the first point of contact for NHS Funded Care patient referrals and for notifying Nurse Assessors when new assessments, three monthly and yearly reviews are required
Analytical and Judgement Skills
+ To assist in the development of administrative processes and systems
+ To be responsible for monitoring documentation to ensure relevant data/information regarding assessments and other information are recorded appropriately - either on database, spread sheet or hard copy
Planning and Organising
+ To provide administration support to the Funded Care Team
+ To attend meetings as necessary and to take part an active part in discussions concerning policy and procedure matters
+ To set up as requested, meetings and reviews on behalf of the FCT nursing staff and liaise with other professionals and members of the public to ensure
+ To be responsible for populating and maintaining the database with relevant information generated by Nurse Assessors. To access database for the purpose of producing appropriate reports on a monthly basis and/or as requested by the NHS Funded Care team
Responsibilities for physical and financial resources
The post holder will
+ Ensure the correct personal/organisational and financial data is held for patient package in Broadcare that they update personally and for assisting in running data cleansing exercises to ensure the financial data help can be used by other departments in reconciling monthly payments.
+ Assist with order new stationery/IT supplies for the team and maintaining existing stock of essential items.
+ Have good keyboard skills, which enable documents/reports to be produced quickly and accurately using various IT packages.
Information resources
+ The post holder will be required to demonstrate proficiency in the use of all office IT applications i.e. word processing, e-mail - outlook, accessing of internet, maintenance of excel spread sheets and database management

The post holder is expected to be able to use their initiative although will work under the supervision of a Senior FCT Staff
Partnership Working/Collaboration

The post holder is expected to cooperate with internal and team audit procedures for the purposes of continuous improvement of the Team's work.
