Full Job Description

The Frailty Virtual Ward Team is an exciting, new, and evolving team,and we are actively recruiting to the service. Currently we require 2 administratorsto provide secretarial and administrative support to the clinical team, helpingto ensure the smooth and efficient running of the service.

The role requires a high level of administrative skills, alongside theability to work as part of a multi-disciplinary team providing sympathetic,kind and confident liaison with patients, relatives, and staff, as well as GPsand the wider multi-disciplinary team.

The successful candidate will providean efficient, patient focused telephone and administration service, workingclosely with the nurses and therapist within the team.

The role will include afull range of clerical and administrative duties, including inputting referrals(onboarding) onto SystmOne and supporting the virtual ward co-ordinators in theonboarding, day to day management and discharging of patients from the frailtyvirtual ward.

Main duties of the job

To deliver an effective and competent level of accurate call handling forcallers to the frailty virtual ward in a calm, professional manner.

To provide the first point of contact with telephone / email contacts tothe frailty virtual ward team

To respond to all incoming calls / referrals, to ensure that patients areescalated appropriately to the virtual ward nurses / co-ordinators.

Promptly refer issues / concerns outside of the scope of the postholders responsibility to the co-ordinator.

Receive referrals from a range of professionals includingmulti-disciplinary team members from hospital, community, and primary care teams.

Utilisation of excellent communication skills, especially in stressful ordifficult situations.

Utilise InformationCommunication Technology inclusive of telephony, email, Microsoft office toachieve effective communication with stake holders and staff.

In the event of systemfailures staff are required to initiate appropriate steps in line with standardoperating procedure to support maintaining business continuity.

To be familiar with amanual method of logging calls on appropriate forms when electronic methods areunavailable or inappropriate.

To inform the virtual ward co-ordinator of any issue affecting servicedelivery, at the time the issue is highlighted.

To providepersonal administration and secretarial support for the frailty virtual wardteam to ensure smooth and efficient running of the service.

To maintaina high professional standard in terms of the performance of duties and in termsof general conduct, including maintaining confidentiality.

To workflexibly across organisational boundaries as required and to present at alltimes a positive image of the frailty virtual ward team.

Toprioritise workload to meet agreed timescales.

To take andtranscribe minutes of approved Trust forums, ensuring they accurately reflectthe discussions at the meetings.

To act as apivotal link of the department for all staff.

To collatedata for clinical audit and generate reports using appropriate officeapplications

To organisethe induction for new staff and visitors.

Typing ofclinical letters / medical reports and other correspondence required by theTeam.

To beresponsible for sourcing, ordering, and auditing of resources.

To maintaineffective filing and archiving system.

To maintainhealth and social care electronic patient records.

To processreferrals in to and out of the service on both health and social careelectronic systems.

Communication :

To liaiseand maintain excellent communication skills, both written and verbal, at alllevels within the frailty virtual ward team.

To organisemeetings as directed. These may require contact with multi agencies requiringthe co-ordination of mutually convenient times.

To act asreceptionist as and when required.

To assessand action information received, using own initiative and to refer on toappropriate individual if necessary.

To be thefirst point of contact for clients and offer a professional solution to clientqueries.

To be ableto communicate effectively and professionally both on the telephone and face toface with patients, and other professionals.

ProfessionalDevelopment :

Toparticipate in protected learning time and attend clinical and administrativestaff meetings as required.

Toparticipate in the individual performance review system.

To updateadministrative / computer skills,

  • To have proven clerical experience and / or be able to demonstrate administrative knowledge.

  • To have proven experience of working within an office environment.

  • Experience communicating with clients exhibiting challenging behaviour.

  • Experience of working in a multi-professional team

  • Experience of SystmOne


  • Qualifications

    Essential
  • A good general standard of education

  • Word and Excel

  • Typing to RSA Level or equivalent experience.


  • Desirable

    NVQ III in Business Administration or equivalent experience,
  • Demonstrate excellent verbal and written communication skills with patients, carers and other professionals.

  • Excellent team player.

  • To be able to use own initiative.

  • To be able to prioritise workload in order to meet given deadlines.

  • Advanced keyboard skills and use of Microsoft applications

  • Minute taking

  • Diary maintenance to include both electronic and manual
