Full Job Description

Open only to applicants from Hampshire & Isle of Wight NHS system, which is limited to NHS Hampshire and Isle of Wight, Southern Health Foundation Trust, Portsmouth Hospitals University NHS Trust, Isle of Wight NHS Trust, University Hospital Southampton Foundation Trust, Solent NHS Trust, South Central Ambulance Service or Hampshire Hospitals Foundation Trust.
All Age Continuing Care covers, Continuing Healthcare and NHS-funded Nursing Care as a statutory duty of the Integrated Care Board for individuals with a 'primary health need', including Mental Health, Learning Disabilities, Physical Disability, and Fast Track.
All Age Continuing Care also has Integrated Care Board duties to children and young people (Children's Continuing Care), joint packages of health and social care (where an individual is not eligible for Continuing Healthcare but specific needs have been identified through the Decision Support Tool that are beyond the powers of the local authority to meet on its own), personal health budgets, stand-alone health needs, s117 of the Mental Health Act and Specialist Rehabilitation (Acquired Brain Injury) ., All Age Continuing Care delivers the patient specific Learning Disabilities and Autism Programme Integrated Care Board responsibilities.
In some areas positions will be within an integrated role with duties across Integrated Commissioning or Local Authorities.
Pro-actively support All Age Continuing Care (AACC) Lead Administrator and Business Manager in the provision of business support to the NHS All Age Continuing Care team and (in some areas integrated Quality functions). This will include providing business, secretarial and administrative support.
Completion of activities to ensure that the NHS All Age Continuing Care processes run efficiently and within mandated timescales.
Responsible for completing the administration functions for the referral, assessment and review process (including Complex Care, Continuing Healthcare Assessments, Fast Track referrals, Retrospective applications and Funded Nursing Care).
Responsible for the booking of assessments, including ensuring that the appropriate documentation has been provided and quality checked for accuracy.
Responsible for ensuring the timely arrangement of initial and review assessments for clients including diary management of assessors., 2. In compliance with Part 7 of the Immigration Act 2016, all applicants must be able to communicate fluently in English to an appropriate standard which will be assessed as part of the selection process.
