This post will be joining a collaboration between Dementia UK, and NHS Black Country Health Foundation Trust. Admiral Nurses are specialist dementia nurses who work with and support families and carers of people with dementia. Working collaboratively with other professionals, Admiral Nurses seek to improve the quality of life for people with dementia and their carers. They use a range of interventions including practical, clinical, emotional, and psychological support to help people live positively with the condition and develop skills to improve communication and maintain relationships. We are looking for an exceptionally skilled, Registered Nurse (adult/mental health/learning disability) with a passion for, and demonstrable track record of, providing person-centred and relationship-centred dementia care with a strong sense of advocacy and equalities for this client group. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of bio-psychosocial interventions to improve the experience and quality of life of families affected by dementia., The Admiral Nurse will support, deliver and coordinate relationship-centred dementia care within Black Country Healthcare NHS foundation Trust in order to improve the experience of carers/families affected by dementia[1] with complex needs. In line with the Admiral Nurse Competency framework at a specialist level, the post holder will provide: + Specialist bio-psychosocial assessment and support for people with dementia and families with complex needs + Facilitation of education and training for staff in the delivery of evidence-based dementia care + Support with policy development and quality improvements in relation to dementia care + Consultancy to other health and social care professionals regarding dementia [1] Dementia UK defines this as the person living with dementia and others who may be carers or family members. Admiral Nurses work together with families to provide support, expert guidance & practical solutions.

This ambitious model is multi-agency and multi-disciplinary, opening the door for you to work in a collaborative and flexible way with partners, service users, carers and their families.Detailed job description and main responsibilities The Admiral Nurse Team is one of a range of initiatives being established across the Black Country. The post will be joining a team of Admiral Nurses situated across Wolverhampton, Walsall and Sandwell and Dudley. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses working across The Black Country and beyond. Dementia UK will support you to maintain and strengthen your clinical development as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities. You'll become a welcome part of a network of Admiral Nurses, and will have the opportunity to access relevant post-graduate training and develop professionally, informed by the Admiral Nurse Competency Framework.

The salary range quoted is as per NHS Agenda for Change pay structures. This is based on the number of years' working directly within the NHS at the advertised pay band. Experience gained with other NHS Trusts will apply, but candidates without prior experience working direct for the NHS would normally be appointed at the lowest point unless they can demonstrate the equivalent experience and then be eligible for incremental increases in future years. Successful applicants for clinical posts up to and including Band 4 are expected to hold the national Care Certificate qualification and will be supported to attain this once in post.
