Full Job Description

Role Purpose: The Admissions and Discharge Co-ordinator will work proactively with the inpatient unit to ensure that all plans and arrangements are in place for the patients admission and discharge. They will assist the Referrals and Discharge Clinical Nurse Specialist and the Inpatient Unit Nurse in charge with any other work to ensure patient flow is maintained within the IPU. They are: One Myton Professionalism Respect Value Main Duties & Responsibilities Admission: Work Closely with the referrals and Discharge CNS and the Nurse in Charge to arrange admissions, liaising with the patient, the family and relevant professionals. This includes informing patients and families of key information about the inpatient unit. Communicate with patients, their families and the person who referred this patient, to ensure all parties
understand admission plans, dealing with any queries in a compassionate, effective manner. To play a proactive role following an admission to introduce self and role to all patients, families and carers. Discharge Facilitate safe discharge by coordinating all appropriate procedures with the inpatient unit team and outside agencies, ensuring that all plans are in place prior to the time of discharge, including transport arrangements. Liaise on and facilitate discharge planning for patients with identified continuing care and complex needs. This includes the co-ordination and completion of continuing healthcare applications through collaborative work with the nursing team. Assist with other aspects of care ensuring that all arrangements and equipment needed are in place for nursing and home care. This may include outpatient appointments, input from other hospice teams and any onward referrals to external agencies such as community nurses. Discuss (in preparation for discharge) plans with
the patient, their family and carer, ensuring there is an understanding of these. Ensure a post discharge check-in call is made by the post holder or a member of the Referrals and Discharge (R&D) Team within 48 hours of discharge. Other key features of the role Liaise with Referrals and Discharge CNS to discuss planned admissions To work across sites to ensure equity of support and engagement with nursing team, patients, families and carers. Attend MDT meetings to ensure discharges are effectively communicated and planned, completing all records and facilitating the completion of any relevant actions from such meetings. Act as a resource, advisor and role model to other colleagues in relation to the admission and discharge process.
