Full Job Description

We are looking for a practitioner who has direct experience of working within a multi-agency setting to work to address young people's support needs as they transition to adulthood and may be experiencing difficulties in relation to substances. You will join an experienced and committed team of drug and alcohol workers and leaving care workers, who all share the same ethos around achieving the best possible outcomes for young people. You will carry a manageable caseload of young people and co-work cases alongside social workers to support a smooth transition to leaving care. The Sorted team is co-located within Northumberland Adolescent Service. This is a specialist service developed to provide tailored support specific to adolescents. It is composed of a variety of services including an accommodation service, substance misuse service, youth justice service, exploitation team and the participation team
