Full Job Description

The Brent Sickle Cell and Thalassaemia Centre provides specialist community services to children and adults with sickle cell disease and thalassaemia. It was the first centre of its kind to be set up in the United Kingdom, over 40 years ago. As part of a Pan-London Sickle Cell Improvement Programme, NHS England has provided funding to improve Sickle Cell Community Services in North West London. LNWH are recruiting to 2 new posts, including Community Clinical Nurse Specialist - Haemoglobinopathies. The service will provide nursing care to adults with Sickle Cell in the boroughs of Brent and Harrow.
We are seeking an experienced nurse with great clinical skills from either acute or community settings. Training and supervision will be provided to ensure the knowledge and skills to meet the demands of the role. The service will work closely with the wider community haemoglobinopathy teams, including Central London Community Health Trust and the Haemoglobinopathy Co-ordinating Centre at Imperial NHS Trust to provide an integrated service across NWL ICB., You will work as part of a team of nurses, working across the 2 boroughs, helping to develop the service, building strong connections in the neighbouring boroughs within the project, and working together as a team in offering clinical support to people with haemoglobinopathy conditions at different stages of their lives, from antenatal screening, newborn screening, childhood, through to adults living with a long-term chronic condition. You will have a clinical caseload and have opportunities to continue to develop your own clinical skills.
Your will be based at the Brent Sickle Cell & Thalassaemia Centre, Central Middlesex Hospital. Patients will be supported by the team at Imperial NHS Trust sites of Hammersmith and St. Marys Hospitals, and the team will provide nursing support in patient homes and venues local to their homes across the 2 boroughs.
If you are an experienced nurse, with passion for haematology or haemoglobinopathy looking for an exciting opportunity to expand, develop haemoglobinopathy nursing, and be part of an amazing transformation project, then this is a great opportunity for you!, This is a new post to support the delivery of NHS England Pan-London Sickle Cell Improvement Programme, through West London ICB with investment into improving haemoglobinopathy services in primary and secondary care at London North West University Healthcare NHS Trust and Imperial College Healthcare NHS Trust. The post is a fixed term contract for 24 months.
Provide specialist nursing and support services for adults with sickle cell, thalassaemia and other haemoglobinopathies in community care settings. Assist the Nurse Consultant for Haemoglobinopathies and Community Clinical Nurse Specialist Team Leader - Haemoglobinopathies in reviewing, formulating and implementing strategies to meet service demands, manage appropriate levels of nursing and other staff, if appropriate.
Maintain professional links with health and allied professionals working in the acute and community sector and with statutory and voluntary agencies. Work within the remit of the NMC code of professional conduct., + Provision of specialist, high quality, expert nursing care and support for patients and their families / carers with haemoglobinopathies trait/disease.
+ Deliver evidence based individualised patient care according to patient's changing health care needs.
+ To work autonomously and seek collaborative relationships with others across and within professional boundaries.
+ To provide expert knowledge and advice in own specialist area, ensuring maintenance of clinical excellence underpinned by evidence-based practice.
+ Work with the multidisciplinary team and management team to ensure that the service complies with the national standards for safety, quality and ensure the service complies with the key performance indicators aligned with the NHS England Sickle Cell Improvement Programme.
Main Duties and Responsibilities
Clinical Responsibilities
1. Act as a clinical expert within your sphere of work.
2. Demonstrates an understanding of haemoglobinopathies, the implications of living with a chronic illness and its impact on individuals, families and wider the community.
3. Develop clinical nursing competencies within your expert sphere encouraging the development of new and novel roles within your service.
4. Demonstrate expert knowledge being visible, accessible and using advanced clinical skills within the workplace as appropriate.
5. Carry a clinical case load across primary care settings, provide support for secondary care colleagues.
6. To establish a partnership approach to care, empowering and educating adults, their carers and their families to actively participate in delivery of care - demonstrating, educating, instructing and supervising them in nursing procedures, which may be delegated to within community and hospital setting as required.
7. Provide expert advice on issues related to the speciality internally and external to the organisation.
8. Use knowledge and skills to advise adults, carers and their families on symptom management and care.
9. To support the transition of adolescents to the adult services within the Trust.
10. Liaise with district/ community nurses, and other members of primary healthcare team to ensure ongoing support of the patient, carers and family.
11. Supervise and ensure a consistent approach to nursing across all areas covered, through collaborative working and the development of shared procedures and guidelines, supervising the auditing of practise and development of action plans.
12. To be responsible for the assessment, escalation and management of risk in accordance with the Trust's Risk Management policy using principles of learning organisation.
13. Keep informed of national and Trust policies, especially those affecting service delivery and patient care.
14. Provide clinical cover when required within the specialist services.
Managerial / Leadership
1. Provide support for Community Clinical Nurse Specialist Team Leader Haemoglobinopathies as required.
2. Participate in preparation and delivery of key performance indicators linked to the NHS England Pan-London Sickle Cell Improvement Programme.
3. Promotes a culture that supports patient centred care and patient empowerment.
4. Acts as a role model demonstrating high standards of care and providing clinical leadership to others.
5. Participates in the recruitment and induction of new staff.
6. Collates and interprets quantitative and qualitative data to provide evidence of productivity, outcomes and quality. Can utilise data to develop reports as appropriate.
7. Ensure that all adult haemoglobinopathy activity is recorded on Trust electronic record system to assure contemporaneous record keeping. Ensure accurate data quality data accreditation standards are maintained when inputting data on IT systems.
8. Contributes to the identification of service goals and appreciate how these links with key performance targets and quality indicators.
9. Actively participates in multi-professional meetings, acting as the patient advocate and representing nursing and patient views.
10. Work closely with the other Clinical Nurse Specialists within the Haemoglobinopathy Nursing Team and provide cross cover if /when necessary.
11. Ensure work links to the goals and objectives of the haemoglobinopathy service and the wider Trust nursing agenda through representation on committees / working groups as appropriate.
12. Attends and participates in organising local, national and international networks and forums relating to sickle cell, thalassaemia and related conditions to support best practice within the service and contribute to the development of service strategy.
Education
1. Takes personal responsibility for life-long learning and personal development through clinical supervision, appraisal and Knowledge and Skills Framework and actively engages with learning and development opportunities.
2. Influence the training and development of others (MDT, nursing roles, etc.), in particular supports non specialist staff to care for patients with haemoglobinopathies.
3. Provides specialist education and training to other professionals involved in patient care.
4. Be appraised at least annually and contribute to own personal development plan (PDP).
5. To maintain compliance with all mandatory and statutory training
6. When appropriate act as a mentor for newly appointed Clinical Nurse Specialists.
7. Access clinical and safeguarding supervision
Service Improvement
1. Supports the nursing contribution to service development, ensuring the pursuit of excellence in care.
2. Develops new skills in response to emerging knowledge and techniques.
3. Works across professional boundaries using creative reasoning and problem-solving to facilitate new ways of working to influence improvements in the patient care pathway.
4. Instigates and manages change within a complex environment. Can evaluate the impact of change and any impact on the patient care pathway.
5. Promotes patient and public experience feedback through local initiatives to drive innovation and change.
6. To maintain accurate data relating to the clinical service ensuring strict confidentiality, compiling statistical data to profile the service and inform its future strategic development.
Research & Audit
1. Influences and participates in development of policy and clinical guidelines internally and externally.
2. Work closely with colleagues to develop evidence-based practice within this area of nursing, contributing to the clinical governance agenda.
3. Leads the development and facilitates regular reviews of all protocols and guidelines specific to haemoglobinopathies.
4. Promotes the service and clinical excellence by publishing and presenting innovations, audit and research locally and nationally.
5. Participates in relevant local and national audit for specialist group.
