Full Job Description

The post holder will be an excellent communicator and work well both individually and within a team.

They will carry out planned and unplanned health (nursing) interventions within the continence service.

They will be required to undertake holistic continence assessments, and work with patients to develop individualised treatment programmes to promote continence.

The post holder will hold clinics in both Sunderland and South Tyneside and visit patients in their home when the patient is unable to attend clinic.

They will be a lead nurse for identified care homes.

They will contribute to the delivery of the education programme for both Trust Staff and care home staff.

Informal visits encouraged.

They will work flexibly across Sunderland and South Tyneside whilst being based at based at Clarendon, Hebburn.
