Full Job Description

Your role will be to provide Urgent and Emergency response to Mental Health Referrals from The Rotherham Foundation Trust (TRFT) with whom we work in close partnership. You will undertake full bio-psychosocial assessments and contribute to providing safe and supported discharge plans. You will work closely with TRFT staff to develop their knowledge of Mental Health- including involvement in delivering training and supporting TRFT staff to develop positive working practices which put the patient at the centre of their own care and promote parity of esteem.

We are seeking an enthusiastic and experienced Mental Health Practitioner to join our Hospital Liaison Team., You will be a knowledgeable and a competent practitioner with excellent assessment and risk assessment skills, the ability to work well under pressure and demonstrate the confidence and ability to work autonomously and as a member of the wider MDT. You will have experience of working with adults with complex mental health needs and have a willingness to continue your professional development., Disability Confident
