Full Job Description

Are you a dynamic, enthusiastic, forward thinking Advanced Clinical Practitioner or Advanced Nurse Practitioner in Frailty looking for a new venture with a like-minded clinical team?, To provide ahigh-quality, cost-effective service to the patients of the Practice; focusingon frail and older people, and those with low-level mental health concerns.

You will also work with and triage other patients outside of frail and elderly.

You will beworking as part of a multi-disciplinary team which include, GP Partners, GPs,Advanced Clinical Practitioners, Nurse Practitioners, Paramedic Practitioners,Clinical Pharmacists, Physiotherapists, Practice Nurses and HealthcareAssistants.

You will beresponsible for face to face and telephone consultations with patientspresenting with a wide range of minor illness, urgent and same day requests fortreatment and chronic disease management, providing care for the presentingpatient from initial history taking, clinical assessment, diagnosis, treatmentand evaluation of care.

You will needto demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills. You will need to prioritise and triage theneeds of patients accordingly making any necessary referrals for investigationsin the appropriate manner.

A largepercentage of the role will be involved the care of frail and elderly patientsincluding the responsibility of patients within care and nursing homes.

This will include weekly ward rounds.

Partof your responsibilities will include visiting patients in their home to assesstheir medical condition and frailty review.

You may also be involved in triageof patients., To provide a high-quality, cost-effective service to the patients of the Practice; focusing on frail and older people, and those with low-level mental health concerns.

  • Experience of working with frail and older patients

  • Experience of providing specialised advice in relation to care

  • Two+ years proven record of working as an ACP dealing with patients presenting with a wide range of medical issues

  • Experience of dealing with patients with undifferentiated and undiagnosed conditions

    Previous Experience in General Practice

    Qualifications

    Essential
  • Full and unrestricted NMC (UK) or HCPC registration

  • RGN (degree or equivalent)

  • Prescribing Qualification

  • Qualification in Physical Assessment skills and Frailty

    A qualification in Frailty Care

    Knowledge and Skills

    Essential
  • Evidence of continuing professional development

  • Evidence of clinical knowledge in caring for patients in relevant specialism

  • In-depth knowledge of recent NHS legislation, recommendations for specialist area.
